How often do you make cosmetic changes to an existing character?

For games like Fallout or Skyrim which allow you (with the appropriate DLCs and quests completed) to alter the appearance of your character after creating them, how often do you do so? I never modify anything but the hair, but I do that pretty often.

In Skyrim only once because I found out you could if you have the legendary edition. But aside from that I always keep my original appearance on any game, appearance doesnt matter much if you have a helmet on xD

I think seasonal changes say that happens with game's season and also the promo event skins and the themes are worth adding. i often make use of them for the design.

The closest I have ever come to making cosmetic changes in a game is changing Links tunic in Ocarina Of Time.
